The well-known problems of quantum measurement, origin of classical properties and realist interpretation are shown to be due to some errors and missing points in the common understanding of quantum mechanics. Thus, the existing theory of measurement is incorrect because it disregards the disturbance by remote identical particles. Quantum mechanics must be modified to obey the cluster separability principle. For classical properties, the focus on a sharp trajectory is wrong because such trajectory is just a mathematical figment. Classical properties have to do with high entropy. As for the realism, the fallacy is the belief that values of observables are the only possible properties of a quantum system though each of them is a property of some system-apparatus composite. Objective properties are defined by preparations. A systematic reformulation of quantum mechanics correcting all these errors has been given.
